1. Hearing Screening & Red Flags (0–12 Months)
Most newborns undergo newborn hearing screening in the maternity hospital via Otoacoustic Emissions (OAE) or Auditory Brainstem Response (ABR). Hearing is fundamental for speech development:
- 0–1 Month: Baby startles (Moro reflex) or blinks to sudden loud clapping sounds; calms down when hearing a familiar gentle voice.
- 3–4 Months: Turns eyes and head toward sound sources; pauses sucking when music or speech begins.
- 6–9 Months: Accurately localizes soft sounds out of direct sight; responds to their own spoken name.
- Hearing Red Flags: Not responding to loud sounds by 1 month; not babbling consonants by 9 months; not turning to their name by 12 months. Request a formal audiology assessment immediately.
2. Physiological Strabismus (Eye Crossing): Normal vs. Red Flag
3. Blocked Tear Duct (Dacryocystitis) & Eye Discharge
Up to 20% of newborns are born with an unopened membrane over the nasolacrimal tear duct (valve of Hasner), causing constant watery tearing and sticky yellowish crusts:
- The Crigler Lacrimal Sac Massage: Wash hands thoroughly. Place clean index finger between the inner corner of baby's eye and the bridge of the nose. Apply gentle, firm downward stroking pressure toward the nostril 3–4 times daily. Over 90% of ducts open spontaneously by 9–12 months with massage.
- Clean with Sterile Saline: Wipe discharge using a sterile cotton pad soaked in sterile 0.9% saline, wiping strictly from outer corner inward toward the nose. Use a fresh pad for each eye.
- 🛑 Dangerous Folk Myth: Never squirt breastmilk into an infant's eye! Breastmilk is rich in sugars and nutrients that act as an ideal culture medium for bacteria, turning sterile blocked tears into severe bacterial keratitis.
