1. Oral Hygiene Begins with the Very First Tooth!

The American Academy of Pediatric Dentistry (AAPD) and European Academy of Paediatric Dentistry (EAPD) advise: brushing must begin as soon as the very first primary tooth breaks through the gumline (usually around 6 months):

  • Use an age-appropriate soft-bristled baby toothbrush with a small compact head.
  • Brush twice daily: once in the morning after breakfast and crucially at night right before bedtime.

2. Fluoride Toothpaste Dosing: The Grain of Rice Rule

Under 3 Years: Grain of Rice Smear
Use a tiny smear of fluoride toothpaste (1,000 ppm F) no larger than a dry grain of rice (approx. 0.1 mg fluoride). This tiny amount strengthens infant enamel while being completely safe if swallowed.
Age 3 to 6 Years: Pea-Sized Amount
Once child learns to spit out (around age 3), increase to a pea-sized dab of fluoride toothpaste (1,000–1,450 ppm F). Encourage spitting, but do not rinse with copious water to let fluoride protect enamel.

3. Preventing "Baby Bottle Tooth Decay" (Early Childhood Caries)

🛑 Bedtime Bottle Warning:
Never put an infant or toddler to bed with a bottle containing milk, infant formula, sweetened tea, or fruit juice.
  • During sleep, saliva flow decreases dramatically. Milk sugars pool around upper front teeth for hours, feeding acid-producing bacteria (Streptococcus mutans) and rapidly destroying primary teeth down to the roots.
  • Only plain water is safe in a crib or bedtime cup once teeth have been brushed for the night.

4. The First Dental Visit: "By Age One"

Schedule your baby's first dental checkup by their first birthday or within 6 months of their first tooth erupting. The first visit establishes a "dental home", screens for enamel hypoplasia, and ensures early guidance on thumb-sucking and pacifier weaning.
